Summary of Terms

A patient is under the clinical care and control of his or her physician or surgeon who may also involve other doctors in treatment if appropriate. OneWelbeck Group staff provides care and treatment under the doctor’s clinical instructions. The physician/surgeon who admits you is not usually an employee of this company and will normally invoice private patients separately for his/her services. The consultant is responsible for arranging the patient's consent for specific medical or surgical treatment if it is required.

For the purpose of this contract, “centre” and “clinic” shall mean any hospital or facility owned by Welbeck Health. Unless otherwise stated, this contract is between the centre providing the services and the patient. The contract shall be governed by and construed in accordance with English law and English courts shall have exclusive jurisdiction. The patient or patient’s representative signature confirms acceptance of the terms and conditions of admission.

Except as expressly provided elsewhere in these terms and conditions, a person who is not a party to this agreement shall not have any rights under the Contracts (Rights of Third Parties) Act 1999 to enforce any term of this agreement.

The patient is liable for all personal expenses and any charges not settled by an insurer or sponsor. If there are delays in settlement, there may be penalties, for example the charging of interest, the revoking of discounts, and any further collection costs incurred by OneWelbeck Group (including fees bill by collections agencies) may be added to the account, for which the patient will be liable.

The patient undertakes that it is their responsibility to pay for the centre’s service whether or not they have private medical insurance or an arrangement with any other third party. The patient accepts full responsibility for settlement of their account in the event that their insurer or sponsor do not pay or refuse to pay their account in full. The patient consents to their credit card details being held on file by Welbeck Health and accepts that Welbeck Health may debit this card with prior notification in the event that any shortfall remains unpaid. Where the centre has a direct billing arrangement with their private medical insurer, The undertakes to provide all the necessary information to the clinic and insurance provider to enable this arrangement to be utilised.

We require payment in full or a deposit at or before the time of treatment or consultation. The value of the deposit required will vary with treatment and at the discretion of the centre providing treatment.

The patient has been notified of the Data Protection Act 2018 as it relates to their data processed by OneWelbeck Group and its parent company Welbeck Health as found in our privacy policy.

The patient declares that their GP recommended the specialist treatment and that to the best of their knowledge and belief the information given on this form is true and complete. The patient authorises the clinic to submit claims relating to their insurer on he patient’s behalf.

A summary of the patient health record will normally be sent to the patient's GP after discharge from the clinic. If the patient were not referred to the clinic by their GP we require consent to enable us to send their health record to them. The patient consent is deemed given unless indicated on this form. If the patient withholds consent, a summary will be given to them for your retention and future reference.

The clinic is responsible to audit clinical records for quality assurance reasons, which may include the patient's health record. The patient consent is deemed given unless indicated on this form. Patient data will be treated strictly in line with National CQC Standards and will be subject to confidentiality according to the Data Protection Act 2018.

The centre does not accept any responsibility whatsoever for the loss of any cash or valuables belonging to patients or visitors.
